Eligibility
Eligibility for services is determined by the Department of Mental Health (DMH), Springfield Regional Office, located at 1515 E. Pythian St., Springfield. Call 417.895.7400 and ask for an Intake Specialist.
To be eligible for service coordination an individual must have a developmental disability defined by the state of Missouri as:
• a mental and/or physical disability that began before the age of 22; and
• that is likely to continue indefinitely; and
• results in substantial functional limitations in two or more of the following areas of major life categories:
• living and working independently
• communication
• self-direction
• learning
• mobility
• self-care
